  7. Handicapped children must receive continuous coverage on their parents policy after they reach normal age that would end their coverage.

  8. Insurance companies in the state of Arizona can not deny claims for physical therapy or occupational therapy out of network because of lack of referral.

  9. Ambulance service.

  10. In the event that someone has a disabling, chronic, degenerative, or life threatening disease then arrangements must be made that they have a standing referral at all times.

  11. Breast reconstructive surgery following a mastectomy.

  12. Group insurance for a company with more than 50 employees can not place a lifetime limit on mental health care.

  13. Covered surgical procedures must be covered regardless if the medical treatment facility is in or out of network.

  14. Prescription drugs have to be covered for an additional 60 days after the insurance company has provided written notice that the drug has been removed from their formulary.
